Class Problem

"The Problem data structure describes relevant information related to each problem documented for the behavioral health client during the episode of care. This may include active (or relevant inactive/resolved) problems from previous episodes of care as well as any documented during the current encounter." - HL7 Behavioral Health Record DAM R2, Problem

Note that this class corresponds to the FHIM HealthConcern class, and therefore, the HealthConcern may be substituted for this class. Thus, the attributes are all marked as derived.

"This data element describes the diagnoses (e.g. Principal Diagnosis, axis-I-problem, axis-Ii-problem, axis-IV-problem, axis-V-problem, physical Problem, etc.) for a behavioral health client and can be used to express behavioral and/or physical health problems. This element draw its values from a standards-based Problem value set based on the exchange requirements (e.g., representing Problems in a Problem List for exchange will use SNOMED-CT, whereas for billing, ICD, CPT, HCPCS (Healthcare Common Procedure Coding System), DSM, etc. code systems are appropriate). The Problem can be captured in an EHR during the clinical documentation process using values from one code system (i.e., SNOMED-CT) and then used during the exchange of a Summary Document such as CCD. This element can also be re-used for billing purposes and exchanged in the appropriate required format (ICD, CPT, HCPCS, etc.) using a terminology service." - HL7 Behavioral Health Record DAM R2, Problem.problemCode
